Community Health Nursing Program
Community Health Nurses provide a variety of assessments, referrals,
and home visits throughout Tazewell County.
Perinatal Follow-up
- All infants admitted to OSF High-Risk Nursery receive
home visits at
1-2 weeks after leaving the hospital, 6 months, 12 months, 18 months,
and 2 years.
- Growth and development is monitored at each visit.
- Appropriate referrals are made when physical or
developmental
problems are identified.
WIC Program
- The WIC (Women, Infants, and Children) program is
federally funded
through the Department of Agriculture.
- WIC provides nutrition education, health assessments
and food vouchers
to pregnant, breastfeeding, and post partum women, and families with
infants and children under the age of 5. For Internet education

- Clients must reside in Tazewell County, be within the
required federal income
guidelines, and have a nutritional or medical reason to qualify.
- Appointments are required. Call extension 230 or 233.
- Certified Lactation Counselors and Breastfeeding Peer
Counselors are
available to help breastfeeding women with questions or concerns.

Family Case Management
- Family Case Management is funded by the Illinois Department of Human
Services (IDHS)
- The program provides public health nursing services to all medically
eligible pregnant women and children up to the age of 12 months. These
services include home visits with an emphasis on prevention. Health
education, growth and development assessments, and referrals to needed
services are provided.
- The goal of the program is to improve the health of pregnant women and
their children.
Pregnancy Testing/Prenatal Classes
- Free pregnancy testing is available by appointment.
- Prenatal nursing staff assists pregnant clients with initiation of a
IDHS Medical Card, finding a physician, and prenatal education.
- Prenatal classes are scheduled monthly in Creve Coeur, Pekin, and
at the Health Department.
- Classes are free and open to the public.
All Kids Healthcare Programs
- State health insurance applications are available for families.
